The CMU Cervical Cancer QoL questionnaire was developed by A/Professor Dr. Kittipat Charoenkwan, Department of Obstretics and Gynecology in 2018This self-rating questionnaire comprises four functional domains  (35 items in total) i.e. gastrointestinal (9 items), urinary (7 items), lymphatic (4 items), and reproductive/hormonal (15 items). The assessment is based on a 5-point
rating scale ranging from 0 to 4, where 
0” represents no symptom and 4” represents the most severe symptom.
